The IOM's quality initiative has gone through three stages since its genesis 6 years ago. In its first phase, the National Roundtable on Health Care Quality, convened by the IOM, highlighted serious problems in the quality of care and noted them to be pervasive throughout the country. The work of this group, heightened awareness of the overuse, misuse, and underuse of health care services that harm large numbers of Americans every day.
