In addition, good knowledge of local patterns of infiltration is required to achieve complete excision. All tumors may show an irregular, often asymmetric, mainly horizontal subclinical infiltration with a wide extent. When deep infiltrations are present, they are also often asymmetric and hard to determine before excision. Until recently these tumor outgrowths have only been detectable using histologic evaluation, but 3D histology is highly sensitive and can detect even very small tumor infiltrations. Very narrow margins can thus be taken for excision, which helps spare healthy skin as only a subsequently targeted re-excision will be necessary to remove all of the asymmetric tumor infiltrations. Therefore the resulting defect will be as small as possible and as large as necessary, and can be adapted to the requirements for aesthetic surgical defect closure. 3D Histology Evaluation of Dermatologic Surgery compares the most commonly used methods in order to give the reader a graphical overview of the current state-of-the-art. The basic principles of various surgical methods and histological workups of the complete 3D tumor margins are described and extensively illustrated to allow better understanding.
